Medicare Part B Enrollment Forms and Resources at Your Fingertips

medicare part b enrollmentIf you didn’t sign up for Medicare Part B during your initial enrollment period, but now you decide you want it, you may have to wait to sign up during other enrollment periods throughout the year. And, depending on your circumstances, you may also have to pay Part B penalties or higher premium payments every month for as long as you have Medicare Part B.

Enrollment Instructions

If you already have Medicare Part A and now you want to sign up for Part B, fill out the Application for Enrollment in Medicare – Part B (CMS 40B) and return it to your local Social Security Office. The instructions are found on the form.

If you do not have Medicare Part A but you want Part A and Part B , you will need to apply for both (Original Medicare) at your local Social Security office at 1-800-772-1213; TTY 1-800-325-0778 (7 a.m. – 7 p.m., local time, Monday – Friday).
